Error while logging in. Failed login quota. Why does it happen?

I have found that sometimes when the login fails I can hit the page refresh button causing my browser (Firefox) to re-send the login data & that gets me in. It's how I got in to post this.

Thanks for the hint. Worked for me today (after second attempt).
