That can't be right... it shows Canada as 0 - 4% :s I would have thought that the U.S. is much higher too.
I wonder if they're taking climate into consideration?

So the climate is considered also, yes. The CIA Factbook has Canada at 4.6% and the US at 18%, so that's pretty similar...

At present, the "Island" of Loihi has a land surface area of 0 square kms.
Where or what is it?

I'm gonna guess a volcanic island chain yet to emerge - perhaps near iceland?
